Le houblon (Humulus lupulus) pour une approche nutraceutique multifonctionnelle du syndrome métabolique

Le syndrome métabolique (SM) se définit par la présence d’au moins trois facteurs de risque cardiométaboliques, notamment l’obésité, la dyslipidémie, l’hypertension et une altération du métabolisme du glucose. Sa physiopathologie repose sur des mécanismes interconnectés tels que l’insulinorésistance, l’inflammation chronique et le dysfonctionnement endothélial, ce qui rend indispensable une approche thérapeutique multifactorielle.1–4 L’augmentation mondiale du SM, associée à des habitudes de vie défavorables, a accéléré le recours aux agonistes des récepteurs du peptide-1 de type glucagon (GLP-1 RA). Ces derniers améliorent le contrôle glycémique et induisent une perte de poids en activant pharmacologiquement les récepteurs du GLP-1. Malgré leur efficacité, ces médicaments sont fréquemment associés à des effets indésirables gastro-intestinaux et à une perte de masse maigre, soulignant la nécessité de stratégies alternatives plus physiologiques. 5–8

Parmi les solutions nutraceutiques émergentes, les extraits de Humulus lupulus (houblon) suscitent un intérêt croissant en raison de leur composition spécifique en acides amers. Les α-acides activent les récepteurs intestinaux du goût amer (TAS2R), favorisant la libération endogène de GLP-1, de CCK et de PYY, hormones impliquées dans la satiété, la vidange gastrique et la régulation de l’appétit. 9  Leurs dérivés isomérisés (iso-α-acides) activent à la fois les PPAR-α et PPAR-γ, régulateurs clés du métabolisme des lipides et du glucose.10-13

SelectSIEVE® HopE (développé par ROELMI HPC) combine ces deux fractions bioactives grâce à un procédé standardisé centré sur une extraction du houblon au CO₂ supercritique et une isomérisation thermique contrôlée. Ce procédé garantit l’obtention reproductible d’un ingrédient, efficace à une dose quotidienne de 200 mg, qui contribue à la santé métabolique en stimulant simultanément une production entéroendocrine physiologique et une modulation des PPAR hépatiques.

Dossier d’efficacité

  • Étude in vitro

Afin de valider son mécanisme d’action, une étude in vitro a été menée, reproduisant fidèlement les conditions digestives auxquelles l’ingrédient est confronté. Les résultats ont démontré que SelectSIEVE® HopE et ses 2 fractions bioactives modulent plusieurs voies intestinales et métaboliques impliquées dans la prise en charge du syndrome métabolique.

En ce qui concerne la libération d’incrétines, SelectSIEVE® HopE a augmenté de manière significative la sécrétion de GLP-1 de 51,7 % par rapport au contrôle négatif, confirmant le rôle des acides α dans l’activation de TAS2R.

La libération de CCK a presque doublé (+98,4 %) et la sécrétion de PYY a également augmenté de 22 %, avec une significativité statistique élevée. Concernant la modulation des PPAR, SelectSIEVE® HopE a significativement augmenté l’activation de PPAR-α (+63,5 %) et de PPAR-γ (+environ 40 %), la contribution prédominante provenant de la fraction iso-α-acide. L’observation d’effets synergiques entre les deux fractions, notamment sur la sécrétion de CCK et l’activation de PPAR-α, justifie pleinement l’utilisation de la formulation combinée plutôt que de ses composants individuels.

  • Essai clinique

Une étude clinique randomisée, en double aveugle et contrôlée par placebo, a été menée auprès de 66 sujets caucasiens en surpoids présentant un syndrome métabolique diagnostiqué selon les critères de l’IDF*. Les critères d’évaluation principaux comprenaient des paramètres anthropométriques et des marqueurs du métabolisme du glucose et de la sensibilité à l’insuline.

  • Mesures anthropométriques

Le tour de taille a diminué progressivement et de manière statistiquement significative dans le groupe actif, avec une réduction de 2,0 cm en trois mois. Le poids corporel a diminué de 82,0 kg au départ à 80,8 kg après 84 jours, soit une réduction de 1,2 kg, et l’IMC a également montré une réduction statistiquement significative d’un point dans le groupe actif, les valeurs restant essentiellement inchangées dans le groupe placebo.

  • Résultats métaboliques

L’insulinémie à jeun a diminué de 10,5 % après 84 jours par rapport à la valeur initiale, une différence significative ayant été observée dans le groupe actif tant entre T0 et T84 que dans l’interaction traitement-temps.

Une évolution similaire a été observée pour l’indice HOMA-IR, un indicateur validé de la résistance à l’insuline, dont les valeurs sont passées de 3,12 à l’inclusion à 2,78 après 84 jours, avec une significativité statistique complète. Le groupe actif a nécessité moins d’insuline pour maintenir l’homéostasie glycémique, grâce à une sensibilité accrue à cette hormone. Concernant l’HbA1c et l’indice d’adiposité viscérale, une tendance positive a été observée dans le groupe actif, contrairement au groupe placebo, qui n’a présenté aucune modification pour l’un et une augmentation pour l’autre. Le profil de sécurité et de tolérance était excellent, aucun événement indésirable n’ayant été rapporté pendant toute la durée de l’étude.

  • Conclusions

Les résultats combinés in vitro et cliniques ont permis d’apporter des éléments mécanistiques clairs et des résultats cliniquement significatifs chez l’homme, confirmant l’efficacité de SelectSIEVE® HopE et en faisant un ingrédient unique pour soutenir la gestion du poids et le contrôle durable du syndrome métabolique.

SelectSIEVE® HopE: An innovative Nutraceutical Ingredient for the Management of Metabolic Syndrome

Humulus lupulus as a Multifunctional Nutraceutical Strategy for Metabolic Syndrome

Metabolic syndrome (MetS) is defined by the presence of at least three cardiometabolic risk factors, including obesity, dyslipidemia, hypertension, and impaired glucose metabolism. Its pathophysiology is sustained by interconnected mechanisms such as insulin resistance, chronic inflammation, and endothelial dysfunction, making a multifactorial therapeutic approach essential.1–4 The global rise in MetS, driven by unhealthy lifestyles, has accelerated the use of glucagon-like peptide-1 receptor agonists (GLP-1 RAs), which improve glycemic control and induce weight loss by pharmacologically activating GLP-1 receptors. Despite their efficacy, these drugs are frequently associated with gastrointestinal adverse events and loss of lean body mass, highlighting the need for complementary physiological strategies.5–8

Among emerging nutraceutical solutions, Humulus lupulus (hop) extracts have gained increasing attention due to their bitter acids. α-acids activate intestinal bitter taste receptors (TAS2Rs), promoting the endogenous release of GLP-1, CCK, and PYY, hormones involved in satiety, gastric emptying, and appetite regulation.9 Their isomerized derivatives (iso-α-acids) activate both PPAR-α and PPAR-γ, key regulators of lipid and glucose metabolism.10-13

SelectSIEVE® HopE  (developed by ROELMI HPC) combines both bioactive fractions through a standardized process integrating supercritical CO₂ hop extraction with controlled aqueous thermal isomerization, providing a reproducible ingredient that supports metabolic health at a daily dose of 200 mg, by simultaneously enhancing physiological enteroendocrine signaling and PPARs modulation.

 Efficacy Dossier

  • In Vitro Study

To validate its mechanism of action, an in vitro study was conducted, accurately reproducing the real digestive conditions encountered by the ingredient. The results demonstrated that SelectSIEVE® HopE and its individual fractions modulate several intestinal and metabolic pathways relevant to the management of metabolic syndrome.

Regarding incretin release, SelectSIEVE® HopE significantly increased GLP-1 secretion by 51.7% compared with the negative control, confirming the α-acids role in TAS2R activation.

CCK release was almost doubled (+98.4%), and PYY secretion was increased as well, with a +22%, and high statistical significance. Regarding PPARs modulation, SelectSIEVE® HopE significantly increased both PPAR-α (by 63.5%) and PPAR-γ activation (by approximately 40%), with the predominant contribution deriving from the iso-α-acid fraction. The observation of synergistic effects between the two fractions, particularly regarding CCK secretion and PPAR-α activation, provides a strong mechanistic rationale for the use of the combined formulation rather than its individual components.

  • Clinical Trial

A randomized, double-blind, placebo-controlled clinical study was conducted on 66 Caucasian overweight subjects diagnosed with metabolic syndrome according to IDF criteria*. The primary endpoints included anthropometric parameters and markers of glucose metabolism and insulin sensitivity.

  • Anthropometric Measures

Waist circumference showed a progressive and statistically significant reduction in the active group, with a   -2.0 cm in three months. Body weight declined from 82.0 kg at baseline to 80.8 kg after 84 days, with a reduction of 1.2 kg. BMI also showed a one-point statistically significant reduction in the active group, with values remaining essentially unchanged in the placebo group.

  • Metabolic Outcomes

Fasting insulin levels had fallen by 10.5 per cent after 84 days compared with the baseline value, with a significant difference observed for active group both between T0 and T84  and in the treatment-by-time interaction.

A similar pattern was observed for HOMA-IR, a validated index of insulin resistance, with values dropping from 3.12 at baseline to 2.78 after 84 days and full statistical significance. The active group required less insulin to maintain glucose homeostasis, thanks to increased sensitivity to this hormone. For both HbA1c and Visceral Adiposity Index a positive trend was observed in the active group, in contrast to the placebo group, which showed no change in one parameter and an increase in the other, respectively. The safety and tolerability profile were excellent, with no adverse events reported throughout the entire study period.

  • Conclusions

The combined in vitro and clinical results determined a clear mechanistic rationale, and clinically meaningful outcomes in humans, confirming the efficacy of SelectSIEVE® HopE and making it a game changing ingredient for supporting weight management and the sustainable control of metabolic syndrome.
